Membership Services of ZIMA

• 24/7 Direct Connectivity:
  You’ll have direct access to the Physician via office phone, personal cell phone (for after-hours needs), email, or text


• Priority Scheduling:
  Guaranteed same day “sick call” appointments Monday through Friday, with minimal to no wait times. Weekend appointments are available subject to the Physician’s availability.


• Continuity of Care:
  In the event the Physician is unavailable, he will personally coordinate the Patient’s referral to a local urgent care or emergency facility and manage the coordination of the Patient’s treatment.


• Hospital Care Coordination:
  Should hospitalization be necessary, the Physician will provide care coordination and advocacy from the moment you arrive at the emergency department until you are discharged, communicating directly with the hospitalist team on your behalf.


• Administrative Support:
  Expedited completion of all paperwork, including specialist referrals, prescription requests, and work-related forms.


• Personalized Appointments:
  Your membership includes focused visits with the Physician to review your health, discuss results, and plan care.



• Care Coordination and Availability:
  Membership Services are provided personally by the Physician in accordance with the terms of this membership. At times, the Physician may be unavailable due to professional obligations, scheduled time away, or unforeseen circumstances.


• When the Physician is unavailable and no covering provider is assigned, he will continue to oversee coordination of your care. If urgent medical attention is needed, the Physician will assist in arranging a referral to a local urgent care center or emergency department and communicate with the treating providers to help maintain continuity of care.


• To ensure seamless coordination, the Physician may share relevant portions of your medical history and current treatment information with the providers involved in your care. All patient information will be handled in accordance with applicable privacy laws and HIPAA regulations.
